When requesting a date for your event, please download the forms below and save to your computer, forms are fillable and can be filled in, and saved right on your computer, then email the form to Frank Robusto for approval of your date and event.

• When approved, the rental form and/or alcohol permit will be emailed to you for an E-Signature and will come back to us automatically after you acknowledge your agreement to the terms and approve your E-Signature (instructions come with the email).

At the same time, a link to Zeffy for payment will be provided. A confirmation of your date will be sent to you via Zeffy.

If you prefer to send in your request via mail, please download and print the following form(s), submit with your payment made out to Heritage Square Museum and send to Frank Robusto / Heritage Square, 2509 Lake Rd, Ontario, NY 14519. A confirmation email will be sent to you when received and accepted. If you do not receive this email or have questions, contact Frank Robusto at the number above.
